OFFICE POLICIES

We take pride in the appropriate reservation of your procedural date and time! Our priority is to schedule procedures that can be attended with the utmost care.

A. CONSULTATION FEE

• The consultation fee is non-refundable but can be applied to the treatment / surgery if booked on the day of the consultation.
• Rescheduling with at least 24 hours’ notice: we will accommodate you for another day up to 2 times without penalty. After that, your internal account will be charged an extra $150 fee.
• No show or same day cancelations will result in an extra $150 charge to your internal account.
• All balances in your internal account must be paid prior to scheduling any future appointments. 

B. SURGICAL PROCEDURES

Surgery scheduling requires careful planning and coordination between our office, the Surgery Center, and their operating room staff, as well as your anesthesiologist, if applicable. In addition, special medical instrumentation is prepared and sterilized for each individual procedure. Therefore, please understand the importance of respecting our policies as they relate to surgical procedures:

Surgery Deposit
A 20% fully refundable deposit is required to reserve your surgery date. The remaining balance is due 2 weeks before the surgery date by cash or credit card only without exceptions.

Cancellation Fee
If the surgery is cancelled within the 4 weeks from the surgery date, the 20% deposit becomes non-refundable.

Revision Procedures
In the event of a revision procedure is required, the surgeon’s fee may be waived (if revision is within 12 months of the surgery date). Please note the patient will be responsible to pay facility fees and/or anesthesia fees.

C. NON-SURGICAL PROCEDURES

• Cancellations or no show on the day of the procedure, will result in a charge to your account of 50% of the total cost of the procedure.
• Scheduled treatments on prepaid packages will result in debit of treatment from series if appointment is cancelled on the day of treatment.
• All balances must be paid prior to scheduling any future appointments.

D. LATE ARRIVALS

In respect for both our patients and providers time, we kindly ask for all patients to arrive on time for their appointment. This ensures that all our patients will receive an exceptional experience at our practice. Please note that any patients with an arrival time of 15 minutes past their scheduled time will result in a rescheduling/cancellation of their appointment. We thank you for your understanding in this matter.

E. COMMUNICATION BY EMAIL AND/OR TEXT

It may become useful during treatment to communicate by email, text message (e.g., “SMS”) or other electronic methods of communication. Be informed that these methods, in their typical form, are not confidential means of communication. If you use these methods to communicate with our office, there is a reasonable chance that a third party may be able to intercept those messages.

I consent to allow the office of Dr. Jose Rodriguez-Feliz and its affiliates to use email and mobile phone text messaging to transmit to me the following protected health information:

• Appointment Reminders
• Health Related Information
• Educational / Informational Material

F. VIDEOTAPE AND PHOTOGRAGH FOR CONSULTATION OR TREATMENT FOLLOW-UP

I hereby irrevocably consent to and authorize the use and reproduction by Jose Rodríguez-Feliz, MD or Soléstica, a Florida limited liability company ("Company"), and its affiliates, or anyone authorized by any of them, of any and all photographs, electronic images, or video footage of me taken by them, or that they have in its possession, provided either by me or by a third party (collectively, Images) for the purpose of an evaluation / consultation or follow-up on any treatment / surgery performed by them.
