FINANCIAL POLICIES FOR SURGERY

Surgery scheduling requires careful planning and coordination between our office, the Surgery Center, their operating room staff, and your anesthesiologist, if applicable. In addition, special medical instrumentation is prepared and sterilized for each individual procedure. Therefore, please understand the importance of respecting our policies as they relate to surgical procedures. All fees are calculated from the scheduled surgery date. All timelines are calculated on actual days, not business days.

A. SURGICAL PROCEDURES

Surgery Deposit

• A 20% fully refundable deposit (up to 30 days) is required to reserve your date. The remaining balance is due 15 days before the surgery date by cash, credit card or wire transfer. Bank fees apply to all wire transfers. No checks accepted.

Cancellations

• More than 30 days: 20% deposit is refundable
• Less than 30 days: 20% deposit is non-refundable unless a diagnosed medical condition, documented by a medical provider, precludes you from having surgery in the future. In which case, the practice retains 10% of the deposit.
• Less than 10 days: 50% of the surgery invoice becomes non-refundable unless a diagnosed medical condition, documented by a medical provider, precludes you from having surgery in the future. In which case, the practice retains 25% of the surgery invoice
• No show / same day cancelations: 100% of the surgery invoice becomes non- refundable unless a diagnosed medical condition, documented by a medical provider, precludes you from having surgery in the future. In this case, the practice retains 50% of the original invoice

Rescheduling

• More than 30 days: $500 rescheduling fee (no exceptions)
• Less than 30 days: 20% rescheduling fee is required to secure future surgery dates (no exceptions)

Revision Procedures

• All revisions are subject to 50% of the paid surgeon’s fee, and the full amount for anesthesia and facility fee (no exceptions)

B. CONSULTATION FEE

The consultation fee is non-refundable but can be applied to your surgery if booked on the same day as the consultation. It is not applicable to products or non-surgical treatments.

Rescheduling within 24 hours will result in a rescheduling fee or $100 rescheduling. If you reschedule with at least 24 hours of anticipation, we will waive your rescheduling fee one time. If you reschedule for a second time, the rescheduling fee of $100 applies.

A “no show” and same day cancelations will result in a non-refundable consultation fee.

C. REFUNDS

Refunds will be provided in the original form of payment except for cash payments. Cash payments subject to a refund will be returned with a company check.
